How to Replace a Public-Health Paradigm Without Cutting a Dollar

In mid-January, the Trump administration cut $2 billion in mental health and addiction grants, then restored the money within 24 hours under bipartisan pressure. The episode looked like a rout: Congress and the states had forced a reversal. But the administration's own explanation of the restoration gave the game away.

The dollars came back. The paradigm changed anyway. That pivot is not a one-off. It is the template. Across at least seven federal functions, the administration has dismantled an agency or program and stood up a successor built on different ideological premises, staffed by different people, and pointed at different beneficiaries. The replacement survives budget restorations because it operates through mechanisms Congress and the courts cannot easily reach. OMB Director Russell Vought named the project last fall.

The before-and-afters now span the whole architecture. SAMHSA, the lead federal agency for addiction and mental health, lost more than half its staff and $2.05 billion in state block grants. ^[1] In its place, the administration launched the Great American Recovery Initiative, co-chaired by RFK Jr. and a White House senior advisor.

Federal addiction response stays on the books, but the method has been swapped. ^[2] The Teen Pregnancy Prevention Program saw 53 of its 67 grants canceled and $67 million withdrawn. The replacement was $63 million for abstinence-only education, marriage promotion, and fertility awareness. ^[3]

The CDC's Advisory Committee on Immunization Practices, the body that sets national vaccine recommendations, had all 17 members fired. Kennedy then rechartered the committee to study safety research gaps and COVID-19 vaccine injuries rather than recommend immunizations, and appointed two new members aligned with that mission. ^[4][5] The committee still exists, but it no longer does what it was built to do. The Education Department's Office for Civil Rights was transferred to the Department of Justice. Special education programs moved to HHS. ^[6]

NIH funding tells the same story from the other direction. Congress passed a bipartisan budget law restoring NIH money with a $415 million increase and requiring HHS to fully staff specific centers. ^[7]

But Kennedy and NIH Director Jay Bhattacharya simply redirected the restored grants toward the MAHA agenda: shifting millions to autism studies, cutting research on transgender health and race and gender health disparities, and moving funding from coastal universities to heartland institutions.

The symbolic layer moved in parallel. The administration ended World AIDS Day commemorations for the first time since 1988 and proposed $1.9 billion in cuts to PEPFAR. In its place: Anti-Communism Week and a Border Patrol anniversary observance. ^[8] The mechanism that makes all of this resistant to congressional and judicial correction is straightforward. A court can block an illegal firing or a funding freeze. It cannot rewrite a committee charter. Congress can restore a budget line. It cannot force an agency to spend the money on the same things. Personnel purges, grant-criteria rewrites, and data deletions operate at the level of administrative discretion. They produce the same result without requiring a single dollar Congress can withhold: a different public-health paradigm. The replacement paradigm has identifiable commercial beneficiaries. MAHA advisers have been explicit about this.

The dismantling of evidence-based public health opens a market for the products MAHA's allies sell. ^[9] Which is why the data deletion matters most. Kennedy's HHS has discontinued the Drug Abuse Warning Network, scaled back foodborne illness monitoring to only two pathogens, ceased transgender student surveys, and delayed abortion surveillance reports. ^[10] The systems that would measure whether the new approach works or harms are being removed alongside the old programs.
