Governor Lee ordered a comprehensive third-party review to determine the cause of the failure. This incident follows a failed execution attempt in May involving inmate Tony Carruthers, during which staff struggled for over an hour to secure an IV line. The Tennessee Department of Correction maintains that it followed every step of the state's approved protocol and that the drug used has been reliably effective.

Pike, the only woman on Tennessee's death row, was convicted of the 1995 murder of 19-year-old Colleen Slemmer. Her legal team has since raised concerns regarding the fairness of her original trial, citing fetal alcohol exposure and a history of childhood sexual abuse that they claim the jury never considered. The execution had proceeded only after the Supreme Court of the United States overruled a federal appeals court stay.
