Pike, the only woman on Tennessee's death row, survived two doses of pentobarbital administered at the Riverbend Maximum Security Institution. Witnesses reported she remained conscious and was heard snoring for over an hour before being transported by ambulance to a hospital, where she remains unconscious on a ventilator in critical condition.

Pike's legal team alleges the execution was a form of torture, claiming staff used at least seven needles and failed to properly place IV lines, causing the drugs to enter her body rather than her bloodstream. The Tennessee Department of Correction maintained that it followed all established protocols. This incident marks the second botched execution in the state this year, following a failed attempt to execute Tony Carruthers in May due to IV access issues.

In the aftermath, Prison Commissioner Frank Strada resigned on October 3. Governor Lee appointed former U.S. Attorney Ed Stanton to lead the investigation into whether protocols were followed. The event has intensified the Tennessee gubernatorial race, with Republican candidate Marsha Blackburn advocating for the return of the electric chair to ensure justice is delivered swiftly, while Democratic candidate Jerri Green has called for a moratorium on the death penalty, describing it as inhumane and biased.
