US Hospitals Face Revenue Drops and Medicare Payment Cuts

United States hospitals prepare for significant 2027 financial losses driven by Medicare payment reductions and declining Affordable Care Act coverage.

United States hospitals are preparing for severe financial challenges in 2027, driven by a combination of government payment cuts and a shrinking pool of insured patients. The Centers for Medicare and Medicaid Services adopted a rule to reduce doctor payments for 2027, which will directly impact facility revenues.

Financial pressure is further compounded by a projected rise in uninsured patients as individuals lose Affordable Care Act coverage and state subsidies for Medicaid beneficiaries decrease. J.P. Morgan analysts indicate that declining enrollment in government-run ACA exchanges will likely hinder profit growth across the sector.

Major hospital chains are reporting divergent outlooks. While Tenet Healthcare continues to see patient inflows from the aging baby boomer demographic, other providers face steep losses. HCA Healthcare expects net optional benefits to decline by $550 million to $650 million in 2027, and Universal Health Services projects a loss of $175 million.
