Joe Biden

Ex-president, cancer patient, Trump critic

Completed radiation for prostate cancer in Oct. 2025; son Hunter told the BBC this month the disease has spread to his bones. Campaigning for Democrats while fighting a House cognitive-decline probe.

Where they stand

Joe Biden is re-emerging as the loudest Democratic voice against Donald Trump even as his health and his presidency's legacy face simultaneous assaults from Congress, former allies, and his own family.

His son Hunter told the BBC in early August that Biden's prostate cancer has metastasized into his bones, describing the condition as very painful and very debilitating. Biden completed a five-week radiation and hormone therapy regimen at Penn Medicine in Philadelphia in October 2025, ringing the ward bell to mark the finish. His spokesperson declined to say whether further treatment would be needed.

On the campaign trail, Biden delivered the keynote at the Maryland Democratic Party's Back and Win Summit in late June 2026, branding Trump a loser and accusing his administration of corruption on a scale never seen before in American history. He cited the White House East Wing demolition, the Kennedy Center naming, the Lincoln Memorial Reflecting Pool no-bid contract, and a proposed $1.8 billion compensation fund for January 6 rioters. At a South Carolina Democratic fundraiser in February, he accused Trump of trying to steal the election through prohibitive voting requirements. At the Jesse Jackson memorial in Chicago in March, he warned that the current administration does not share any Democratic values.

The House Oversight Committee, chaired by James Comer, is investigating an alleged cover-up of Biden's cognitive decline and the autopen used to sign over 4,200 clemency orders. Jeff Zients, his former chief of staff, testified that Biden's memory deteriorated and decision-making slowed, and that Jill Biden and an aide requested schedule adjustments to give the president more rest. Biden's former physician Dr. Kevin O'Connor invoked the Fifth. Biden has denied the allegations, asserting in a DOJ lawsuit filing that he personally made all decisions during his presidency.

His circle is fracturing publicly. Nancy Pelosi denied knowing of his cognitive decline before the June 2024 debate and faulted his family and staff. Hillary Clinton called his re-election bid a terrible mistake. Kamala Harris called it recklessness driven by ego. Pete Buttigieg publicly rejected Biden's 2020 messaging ahead of a potential 2028 run. Jill Biden suggested on a podcast this week that Biden may have been drugged before that debate, saying he did not feel well that day and she feared a stroke. Hunter separately blamed the sleep aid Ambien. Biden is also fighting in court to block the DOJ from releasing audio recordings of interviews with his ghostwriter, after a federal judge ruled the redacted materials can go to the Heritage Foundation.

5 focus areas

On their plate

1.
Metastatic Prostate Cancer

Biden completed radiation and hormone therapy for aggressive prostate cancer (Gleason score 9) at Penn Medicine in October 2025. Hunter Biden told the BBC in August 2026 that the cancer has metastasized into his bones, calling it very painful and very debilitating. Biden's spokesperson has declined to say whether further therapy is needed.

2.
House Cognitive-Decline and Autopen Probe

Chair James Comer's investigation has taken testimony from Jeff Zients, who said Biden's memory deteriorated and decision-making slowed; Dr. Kevin O'Connor, who invoked the Fifth; and Karine Jean-Pierre, who said Biden was not the same speaker she first met. Biden denies the allegations and maintains he personally approved all orders, asserting in a DOJ lawsuit that he made every decision during his presidency.

3.
Campaign Trail as Trump's Democratic Foil

Biden delivered the keynote at the Maryland Democratic Party's gala in June 2026, accusing Trump of unprecedented corruption and citing the Reflecting Pool contract, Kennedy Center naming, and a January 6 compensation fund. He spoke at a South Carolina fundraiser in February and the Jesse Jackson memorial in March, each time warning of executive overreach and democratic erosion.

4.
Ghostwriter Audio Legal Battle

Biden is suing the DOJ to block release of audio and transcripts from 2016-2017 interviews with his ghostwriter Mark Zwonitzer, obtained by Special Counsel Robert Hur during the classified-documents probe. Federal Judge Dabney Friedrich ruled the redacted materials can go to the Heritage Foundation, granting a three-week stay for Biden to seek an injunction from the DC Circuit.

5.
Allies and Family Publicly Split Over His Legacy

Pelosi denied early knowledge of cognitive decline and faulted Biden's family and staff. Hillary Clinton called his re-election bid a terrible mistake. Kamala Harris called it recklessness. Pete Buttigieg rejected Biden's 2020 messaging. Jill Biden suggested on a podcast Biden may have been drugged before the June 2024 debate. Hunter blamed Ambien and told the BBC the presidential pardon was easily criticized for good reason.
