Senate Rejects ACA Subsidy Extension Amid Government Shutdown Deal

The United States Senate rejected a Democratic motion to extend enhanced Affordable Care Act (ACA) premium tax credits, making the expiration of these subsidies likely on January 1, 2026. The decision followed a bipartisan deal to end the longest government shutdown in U.S. history, which reopened the federal government but omitted the health care extensions. While Senate Majority Leader John Thune promised a separate vote on the subsidies in mid-December, House Speaker Mike Johnson has not agreed to a matching House vote.

Donald Trump and several Republican lawmakers oppose the extensions, with Trump proposing a system that provides funds directly to citizens for health savings accounts rather than to insurance companies. Other Republicans, including Senator Bill Cassidy, suggested alternatives like pre-funded federal flexible spending accounts. Despite party lines, Representative Marjorie Taylor Greene broke ranks to support the extension, citing rising costs for small-business owners.

The Congressional Budget Office and nonprofit KFF warn that the lapse could lead up to 4.2 million people to drop coverage and cause monthly premiums to more than double for 22 million enrollees. Florida is expected to be disproportionately affected, with the Florida Hospital Association warning that 1 million residents risk losing insurance. Senate Minority Leader Chuck Schumer characterized the Democratic extension bill as the last train to leave the station before premiums skyrocket, while the administration continues to weigh a broader health care overhaul.
